The Building and Contents Coverage and Contents Value Update form, IRM-02 may be obtained from the Insurance and Risk Management Department, and photocopied as necessary.
E. Instruction for Completion of Form
The form IRM-02 should be completed as follows:
- DEPARTMENT - Enter the department name.
- PHONE - Enter the department's phone number.
- BUILDING - Enter the building name.
- CONTACT - Enter the name of the contact person for the building and contents.
- ASSET LOCATION - If the department does not occupy the entire building, enter the floors and/or rooms occupied.
- ADDRESS - Enter the street, on or off campus, and the city where the building is located.
- IS ASSET FUNDING GENERAL, SPECIAL, OR COMBINED [G,S,C] - Specify how the department is funded.
- COVERAGE DESIRED - Specify the coverage desired by checking the blocks beside the building and contents.
- CAMS VALUE OF CONTENTS - The current CAMS value is listed here. If Insurance and Risk Management has this value, it will be filled in before the form is sent to the department.
- YOUR VALUE - Specify the updated value if different from CAMS value.
- VALUE/COVERAGE EFFECTIVE DATE OF CHANGE (Y-M-D) - Enter the date coverage is to begin.
- REMARKS - Enter any details or special remarks concerning the contents or coverage.
- SIGNED - The contact person or authorized person should sign the form here.
- DATE - Enter the date of the signature here.
