The Building and Contents Coverage and Contents Value Update form, IRM-02 may be obtained from the Insurance and Risk Management Department, and photocopied as necessary.

E. Instruction for Completion of Form

The form IRM-02 should be completed as follows:

  1. DEPARTMENT - Enter the department name.

  2. PHONE - Enter the department's phone number.

  3. BUILDING - Enter the building name.

  4. CONTACT - Enter the name of the contact person for the building and contents.

  5. ASSET LOCATION - If the department does not occupy the entire building, enter the floors and/or rooms occupied.

  6. ADDRESS - Enter the street, on or off campus, and the city where the building is located.

  7. IS ASSET FUNDING GENERAL, SPECIAL, OR COMBINED [G,S,C] - Specify how the department is funded.

  8. COVERAGE DESIRED - Specify the coverage desired by checking the blocks beside the building and contents.

  9. CAMS VALUE OF CONTENTS - The current CAMS value is listed here. If Insurance and Risk Management has this value, it will be filled in before the form is sent to the department.

  10. YOUR VALUE - Specify the updated value if different from CAMS value.

  11. VALUE/COVERAGE EFFECTIVE DATE OF CHANGE (Y-M-D) - Enter the date coverage is to begin.

  12. REMARKS - Enter any details or special remarks concerning the contents or coverage.

  13. SIGNED - The contact person or authorized person should sign the form here.

  14. DATE - Enter the date of the signature here.
