North Carolina State University
2011-2012 Graduating Senior
College & Department Surveys

Several colleges, departments, and/or academic programs are inviting their graduating seniors to participate in a brief survey asking for feedback specific to their programmatic efforts. The results of these surveys provide valuable information for programs to use in assessing how well they are meeting their goals for providing the best possible undergraduate education to their students. In sharing your opinions, you will help them identify what they are doing well, and where they need to make improvements.

If you have more than one major, you might be asked to complete a survey for each of them. Individual surveys should take 5 to 15 minutes to complete depending on your major.

By using your Unity log-on to access the survey for your department, your Student ID number will be automatically associated with your answers. We need your Student ID in order to link your responses to other information in your student record so we are able to better understand your answers and at the same time keep this questionnaire relatively short. Be assured that we will protect your identity and keep your responses confidential! The survey data are stored in a secure location, our reports remove all personally identifying information, and your responses are combined with those from other students. With the exception of the final section of the survey, asking for your suggestions on ways to improve the University, individual responses are not available for use by others.

If you have any questions about this survey, please call our office of University Planning and Analysis at (919) 515-4184 or email studentsurveys@ncsu.edu. Thank you for your help with this important effort to learn more about and improve the undergraduate experience at NC State.

As an enhanced security measure, after entering your Unity ID and password you will be taken to a second screen from which you can access the survey.
To avoid 'time out' problems, please set your 'maximum idle time' on the next page (the Office of Information Technology 'authentication' page) to 60 minutes.
Attention Macintosh users: The online survey will not work properly using Internet Explorer with a Mac.
Please be sure to use a browser that is supported by Mac, such as Firefox.
