North Carolina State University

2000 Sophomore Survey

Dear Student,

NC State University is participating in a system of student surveys that will help us evaluate institutional performance. Students surveyed include new students, students in the middle of their undergraduate programs, graduating seniors, and alumni.

Please help us with this effort by taking a few minutes to complete this evaluation form. It includes a "comments" section which you are encouraged to use if you wish to elaborate on an answer. Your answers to all questions are very important. Look for them to be summarized later in your campus newsletter, on the World Wide Web, or in other media.

By using your unity log-on to access this survey, your Student ID number will be automatically associated with your answers. We need your Student ID in order to link your responses to other information in your student record so that we are able to better understand your answers and at the same time keep this questionnaire short. Be assured that all of your responses will be kept strictly confidential!

If you complete this survey by February 25th, your name will be entered in a drawing for $200! You must complete all questions in order to be eligible for the drawing. If you do not reply by February 25, you will be asked to fill out a printed survey form during pre-registration/advising in March and/or by mail.

Thank you for taking the time to complete this survey.

Sincerely,
Chancellor Marye Anne Fox
